When you’re thinking about transitioning, there’s a lot to consider. Hormones, surgeries, your wellbeing—it’s a big journey. But one thing that’s easy to miss early on is your future fertility.

What is fertility preservation?

Fertility preservation is about saving your sperm, eggs, or embryos now, so you have the option of using them later—when the time’s right for you.

If you’re planning hormone therapy or surgery, it’s worth knowing that these treatments can affect your ability to have biological children. So thinking about fertility early in your journey gives you more freedom later on.

What are the options?

The type of preservation that’s right for you will depend on your body, your transition plan, and how you see your future family.

If you’re a trans woman or AMAB (assigned male at birth)

Sperm freezing
Before starting hormones or surgery, you can give a sperm sample, which we’ll freeze and store for future use. It can be used later in treatments like IVF or IUI—with a partner or a surrogate, depending on what’s right for you.

If you’re a trans man or AFAB (assigned female at birth)

Egg freezing
Before starting testosterone, you might want to freeze your eggs. This involves a short round of hormone medication to help your ovaries produce more eggs. We then collect and freeze them, ready to use later.

Embryo freezing
If you have a partner or are using donor sperm, you can also fertilise your eggs before freezing—this creates embryos, which are then stored for future use. Some people choose this route because it can improve the chances of pregnancy down the line.

Why should I think about this now?

We get it—it’s a lot. And family might not be your focus right now. But preserving your fertility before starting treatment means you’re giving yourself options for the future. Here’s why it matters:

  • It gives you choice – You may not want kids now, or ever—but preserving fertility means the decision stays with you.

  • It takes the pressure off – Once you’ve made a plan, you can focus on your transition knowing you’ve got options saved for later.

  • It supports your wellbeing – For some people, knowing they can still have biological children one day brings peace of mind and emotional clarity.

A few things to think about

Cost

Fertility treatment and preservation can be expensive, and not always covered by the NHS. But we’ll talk you through all the costs, and we offer tailored packages and finance options to help manage it.

Timing

The sooner you look into fertility preservation, the more choices you’ll have. It’s best to do it before starting hormone therapy or having surgery that affects your reproductive system.
