If you’re thinking about starting hormone therapy or undergoing gender-affirming surgery, it’s important to know how these steps may affect your fertility. At Care Fertility, we offer fertility preservation options to help you keep the possibility of having a genetically related child in the future—whether that’s months or years down the line.
Here’s what we offer:
Egg freezing
For trans men or non-binary people with ovaries
Egg freezing involves stimulating the ovaries to produce multiple eggs, retrieving them during a short procedure, and freezing them for future use.

You’ll have hormone injections for 10–14 days to stimulate egg production

We monitor your response with ultrasound scans and blood tests

When the eggs are ready, they’re collected under mild sedation

Eggs are frozen and safely stored for use in IVF at a later stage
It’s important to note that you may need to pause testosterone during this process, as it can affect egg development. Our team will talk through what that looks like and offer support throughout.
Egg freezing gives you more choice in the future—whether you want to carry a pregnancy, use a partner or surrogate, or explore donor treatment later on.

Sperm freezing
For trans women or non-binary people with testes
Sperm freezing involves collecting and storing semen samples for future use in fertility treatment such as IUI, IVF, or ICSI.

You’ll first complete routine screening tests for infections and genetic markers

You’ll provide several semen samples, ideally over multiple visits

Once collected, your sperm is frozen and stored in secure cryogenic storage

It can be thawed and used in fertility treatment whenever you're ready
Sperm freezing is typically done before starting hormone therapy, as oestrogen can reduce sperm production. If you’ve already started hormones, we’ll assess whether stopping temporarily could help restore fertility—we’ll support you through that decision, at your pace.

If you’ve already started or completed your transition, you still have options for building a family. Whether you’ve preserved your fertility in the past or you’re now considering treatment with donor eggs, sperm, or surrogacy, we’ll help you understand what’s possible and create a plan that fits your goals and identity.
Here are some of the pathways available:
IVF with your own eggs or sperm
If you froze your eggs or sperm before transitioning, you can use them to try for a baby through in vitro fertilisation (IVF). This involves fertilising eggs with sperm in our lab, then transferring an embryo into a womb—your own, a partner’s, or a surrogate’s.
You might choose this option if:

You’re using a partner’s womb or working with a surrogate
Our team will guide you through every step, from thawing your samples to embryo transfer and pregnancy testing.
Treatment with donor sperm or eggs
If you haven’t preserved your fertility—or don’t wish to use your own eggs or sperm—donor treatment could be the right option. We offer access to fully screened, HFEA-registered sperm and egg donors through our dedicated donor programme.

Surrogacy
If carrying a pregnancy yourself isn’t possible—or isn’t the right path for you—surrogacy may be the best option. This involves creating embryos through IVF and transferring one to a surrogate who carries the pregnancy on your behalf.
We’ll take care of the clinical side, including:

Creating embryos using donor or partner eggs and your sperm

Preparing your surrogate with all necessary medical support

Coordinating closely with your surrogacy agency and legal team

Intrauterine insemination (IUI)
IUI is a simple and less invasive treatment for trans men or non-binary people who have a uterus and wish to carry a pregnancy. It involves placing donor sperm directly into the womb during ovulation to encourage natural fertilisation.
IUI may be a good option if:

You’re ovulating regularly without additional medication

You’d prefer to try a lower-intervention route before IVF
